Para sa kasal na mga Pinay na ginagamit ang apelyido ng kanilang asawa, nakipag divorce now at gustong ibalik or gamitin muli ang kanilang apelyido sa pagkadalaga, these are the things you should do and the documents you should prepare.

Para sa mga dating kasal na mga Pinay at gusto muling gamitin ang kanilang apelyido sa pagkadalaga, ito ang mga additional documents na dapat ninyong ipasa:
1. NSO Birth Certificate. Transcribed Birth Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Birth Certificate are blurred or unreadable.
a. If widowed:
Original and photocopy of NSO Death Certificate of the deceased spouse (if the spouse is Filipino) or Koseki Tohon (if the spouse is Japanese).
b. If annulled:
Original and photocopy of the Philippine Court Order effecting the annulment and annotated NSO Marriage Certificate or Report of Marriage.
c. If divorced:
Original and photocopy of the Philippine Court Recognition effecting the foreign divorce and annotated Marriage Certificate or Report of Marriage.

For holders of brown passports or passports issued before 01 May 1995:
a. Bring original brown passport and photocopy of passport pages 1,2,3,4 and last page.

For holders of Machine-Ready Readable Passports (MRRP) or green passports:
a. Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.
For holders of Machine Readable Passports (MRP):
a. Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.
b. NSO Birth Certificate to verify complete place of birth. Transcribed Birth Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Birth Certificate are blurred or unreadable.
c. Old cancelled passport may be presented in the absence of an NSO Birth Certificate.
For holders of Electronic Passport (E-passport):
a. Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.
