New passport application for adopted Filipino

Kung nais nyong kunan ng panibagong Philippine passport ang inyong adopted Filipino child sa Philippine Consulate Office sa Japan, ito naman ang mga documents na dapat ninyong ihanda.


Bago mag-apply, tiyakin na malinis na ang lahat at tapos na ang adoption processing ng bata, lahat ng documents related on to it registered na at meron kayong hawak na copy. If all of these are completed, then ihanda nyo ang mga documents sa baba upang makapag-apply kayo ng panibagong passport ng adopted child ninyo.

1. Amended NSO Birth Certificate (showing the new adoptive surname) in security paper authenticated by DFA Manila.


2. Old NSO Birth Certificate (using the original surname) in security paper.


3. Certificate of Registration of Adoption (issued by Manila Local Civil Registrar).


4. Photocopy of Authenticated Philippine Court Decree of Adoption.


5. Old passport.


Sa bagong passport na makukuha ng adopted child, lilitaw na dapat dito ang bagong pangalan or surname na gagamitin nya base sa pangalan ng mga umampon sa kanya.
